Falta de saneamento mata 11 mil pessoas por ano no Brasil, mostra Atlas do IBGE

A Doença de Chagas, as diarreias e a disenteria foram as principais causas de morte pelas DRSAIs.

As doenças relacionadas ao Saneamento Ambiental Inadequado (DRSAI) causaram cerca de 0,9% dos óbitos ocorridos no Brasil entre 2008 e 2019, 135 mil pessoas, uma média de 11.200 ao ano.  Os dados estão incluídos na terceira edição do Atlas de Saneamento: Abastecimento de Água e Esgotamento Sanitário, lançada nesta quinta-feira, 24 de novembro, pelo Instituto Brasileiro de Geografia e Estatística (IBGE).

De acordo com o órgão, os números “dimensionam os impactos de um saneamento ambiental desigual e ainda não universalizado no país”. Entre as mortes ocorridas apenas por doenças infecciosas e parasitárias no Brasil, as DRSAI representam 21,7% dos óbitos no período.

As regiões Centro-Oeste (42,9%) e Nordeste (27,1%) são as que registraram os maiores percentuais.

De 2008 a 2019, foram notificados no Brasil 11.881.430 casos de DRSAI, com 4.877.618 internações no Sistema Único de Saúde (SUS). As principais causas de morte foram Doença de Chagas, diarreia e disenteria, que corresponderam a quase 81,5% dos óbitos no período.

Nas regiões Sudeste e Centro-Oeste, dengue, zika e Chikungunya foram a terceira causa de óbitos. Já na região Norte, as leishmanioses foram as principais causas. No Nordeste a principal causa foi a esquistossomose e no Sul, a leptospirose.

Para a geógrafa do IBGE, Daiane Ciriáco a explicação está na falta de tratamento de esgoto. “Esgoto a céu aberto colabora na proliferação dessas doenças, daí a correlação direta entre as enfermidades e o saneamento precário”, disse.

O Atlas faz uma análise territorial dos dados da Pesquisa Nacional de Saneamento Básico 2017 (PNSB), além de outras fontes do IBGE e de outras instituições. Segundo o IBGE, isso permite estabelecer “uma visão articulada das diferenças regionais existentes naquele ano no que se refere à distribuição, abrangência e qualidade desses serviços de saneamento presentes nos municípios brasileiros”.
Esgotamento sanitário

Essa edição apontou que mesmo com as diferenças regionais relevantes nos serviços municipais de esgotamento sanitário e de abastecimento de água, entre 2008 e 2017, houve um avanço no número de cidades cobertas por esses serviços em todas as regiões do país.

O IBGE destacou que a diferença entre a quase universalização do serviço de abastecimento de água por rede geral em contraste com o déficit ainda persistente na coleta de esgoto é uma característica da distribuição dos serviços de saneamento de água e esgoto por municípios. “Enquanto 99,6% dos municípios tinham abastecimento de água por rede geral, apenas 60,3% faziam coleta de esgoto em 2017”, apontou.

Entre as unidades da Federação, somente São Paulo (93,2%), Rio de Janeiro (87,5% ) e Distrito Federal (86,6%) registraram taxas superiores a 85% de domicílios atendidos pelo serviço de coleta de esgoto, conforme demonstrou a Pesquisa Nacional por Amostra de Domicílios Contínua (PNAD Contínua) 2017.

Em todos os estados da região Norte e sete estados da região Nordeste, com exceção de Pernambuco e da Bahia, as taxas de domicílios atendidos com coleta de esgotamento sanitário por rede geral variaram de 9,9% (Rondônia e Piauí) a 54% (Sergipe).

Na avaliação do IBGE, a reduzida abrangência da coleta de esgoto é o que determina o principal tipo de poluição ou contaminação identificada na captação de água doce, exatamente por esgoto sanitário. A falta de investimento no setor de saneamento básico é uma das causas e compromete a qualidade da água distribuída e a eficiência da rede de distribuição, com prejuízos ao meio ambiente e à saúde pública.

A maior abrangência do serviço de esgotamento sanitário por rede coletora em 2017 ficou com o Sudeste. Naquele ano 3,5% dos municípios não tinham acesso à coleta de esgoto. No entanto, no Norte (83,8%) e no Centro-Oeste (57,0%) estavam as maiores proporções de municípios sem rede coletora de esgoto.

“Embora as regiões Norte e Centro-Oeste sejam as menos populosas do país, elas foram as únicas que apresentaram, no período entre 2010 e 2020, crescimento populacional relativo estimado acima da média nacional, de 11%, com taxas de 17,7% e 17,4%, respectivamente”, contou Daiane, alertando para a necessidade de investimentos que atendam à crescente demanda, diante da tendência de crescimento populacional nessas regiões.

Racionamento

O capítulo do Atlas que aborda a integração do saneamento básico com o meio ambiente foi elaborado com a colaboração da Agência Nacional de Águas e Saneamento Básico (ANA). Os mapas se referem aos biomas e às regiões hidrográficas brasileiras nos dois primeiros níveis (macrorregiões hidrográficas e mesorregiões hidrográficas), além do grau de urbanização e da densidade demográfica neste recorte territorial.

A Mesorregião Hidrográfica do Litoral do Rio de Janeiro, por exemplo, tem 98,5% de sua população total em médias ou grandes concentrações urbanas. Em seguida, as mesorregiões hidrográficas do litoral de São Paulo (94,1%), do rio Tietê (89,5%), que são as de maior concentração urbana de São Paulo, e a Mesorregião Hidrográfica do Rio Negro (86,5%), onde está localizada a cidade de Manaus.

No Nordeste, onde está o bioma da Caatinga, que é característico do clima semiárido; nas mesorregiões Hidrográficas Piancó-Piranhas-Açu e Litoral do Rio Grande do Norte e da Paraíba se verifica a ocorrência de racionamento por insuficiência de água nos mananciais em 53,8% e em 46,8% dos municípios com abastecimento de água por rede geral. Já na ocorrência de secas e estiagens, os percentuais alcançam 69,4% e 49,5% nessas duas mesorregiões.

O gerente de Dinâmicas Ambientais e Territoriais do IBGE, André Polly, disse que as causas principais para o racionamento nessas regiões é a ocorrência de secas e estiagem, além da insuficiência de água em mananciais. “Nas duas situações, o principal motivo é a característica natural do bioma. Trata-se de algo sazonal, historicamente registrado”, apontou.

Somente 38% dos municípios do país possuíam planos municipais de saneamento básico em 2017, que visavam as atividades de abastecimento de água e esgotamento sanitário. “Após dez anos do primeiro marco legal do setor, os dados da PNSB 2017 indicam que a regulação do saneamento é insatisfatória e com profundas diferenças regionais”.

Nos serviços por rede coletora de esgotamento sanitário em funcionamento apenas 46,5% dos municípios tinham um órgão regulador. Quando a avaliação se refere aos serviços de abastecimento de água, o percentual chega a 69,5%. Para a universalização dos serviços de abastecimento de água e esgotamento sanitário, somente 11 estados possuíam prazo médio até 2030 ou menos, ano de referência nos Objetivos de Desenvolvimento Sustentável.

A publicação pode ser acessada pela Plataforma Geográfica Interativa (PGI), onde o usuário pode fazer seus próprios cruzamentos e baixar as tabelas e mapas, que trazem os dados para uma dimensão espacial.

De acordo com o IBGE, o Atlas de Saneamento oferece informações atualizadas sobre o saneamento por meio de leitura geográfica das estatísticas incluídas na Pesquisa Nacional de Saneamento Básico – PNSB.
Também marca o compromisso do IBGE em fornecer informação atualizada sobre um tema fundamental da agenda da saúde pública, da sustentabilidade e da própria cidadania na sociedade brasileira.
